feat: replace enterprise templates with MCP-specific issue templates

Remove enterprise_support.md and firewall-request.md (misplaced from
enterprise repos). Add MCP-specific templates:
- mcp_tool_request.md — request new MCP tools with API endpoint details
- mcp_connection_issue.md — report connection/auth/TLS issues
- mcp_api_integration.md — request new REST API integrations

feat(ci): add missing standard workflows from MokoStandards
...
Add cascade-dev, cleanup, repo-health, deploy-manual, and pre-release
workflows to match the standard template set. These are universal
workflows that every MokoStandards repo should include.
